REGISTRATION PROCESS
INFORMATION NEEDED FOR
J.M. TATE HIGH SCHOOL REGISTRATION
- Proof of residence in your name — Gulf Power bill, water bill or mortgage rent statement (cannot use drivers license, or cable bill)
- Shot Records/Blue Card required (Fl. St. 232.032)
- Birth certificate
- Social Security card
- Withdrawal form and school records from previous school
- IEP (if applicable)
- Transcripts

YOU MUST GO TO THE ENROLLMENT SERVICES OFFICE
BEFORE YOU ENROLL IF:
- You live outside of the Tate district.
If the parent and student are living with someone, a notarized address verification form, a copy of the utility bill, and the student transfer request has to be completed.
- Your child lives with another family without a custodial parent in the same residence.
*A notarized letter from the parent giving custody of the student to the people that the student will be residing with must be submitted to the Student Services Office for approval.
If a student is living with someone and is under the age of 18, a supervisory affidavit has to be completed along with a copy of a utility bill, student transfer request, and the notarized address verification form.

Withdrawal
WITHDRAWAL
Advance notice of withdrawal from parents/guardians and/or students is appreciated. Use the withdrawal form process to complete student withdrawal.
Destiny - Utilized to check in books, Chromebooks, and verify any outstanding fines. Students will need to utilize Destiny to check for any fines.
Fines - Exact cash or checks made payable to J. M. Tate High School is accepted.
Once Destiny is cleared, issued items are returned, and any fines paid, then parents/guardians will be given a Focus Withdrawal form to take to the new school.
Upon enrolling at the new school, request the new school send a Records Request form with a good fax number to: aheadley@ecsdfl.us
Upon receiving the Request Form, please allow 24 hours for records to be sent via fax.
Per district policy, we are not permitted to send any records via email.

TESTING
ACT/SAT INFORMATION
Both the SAT and ACT test companies emphasize that the best way to improve scores is to take a challenging academic curriculum which includes higher math (at least through Algebra II) and advanced classes in as many areas as possible. Test preparation classes or resources usually serve to increase awareness of test procedures and questions. They also give hints on how to be more efficient in timed situations and how to reduce anxiety as a result of being more familiar with the test.

KAPLAN PRACTICE TEST ADMINISTRATIONS AND SEMINARS:
Kaplan is a national test preparation company that periodically gives students an opportunity to take a "real" ACT, SAT, or PSAT that is not scored. These sample tests are free, but remember they will encourage you to sign up for an expensive test prep course. Call 1-800-KAPTEST for schedules and reservations. Tests are given at UWF.
BOOKS AND COMPUTER SOFTWARE:
Many companies sell items for test prep. The best materials are usually those produced by the test makers themselves (See links above for on-line purchase). Local bookstores carry resources. Look for ones made by Collegeboard (SAT) and ACT. BE SURE YOU ARE GETTING INFORMATION ON THE NEW VERSIONS OF THE TESTS!
